Common Heart Surgery May Be Unnecessary

The widespread practice of stitching shut a hole in the heart may do more harm than good, by increasing patients’ chance of having a stroke, new research shows.

The hole, a defect called patent foramen ovale, or PFO, often doesn’t cause noticeable symptoms in people who have it. But some surgeons close it when they detect it during the course of unrelated heart surgery.

That practice results in a nearly 2½ times higher chance of postoperative stroke and doesn’t increase patients’ life span, contrary to what was previously thought,according to a study published Tuesday in JAMA, the Journal of the American Medical Association.

Some 25% of adults have the defect, which occurs when a small, flap-like opening between the upper chambers of the heart fails to close during normal fetal development. The condition has been linked to increased risk of stroke, possibly because a blood clot can travel through the opening and up to the brain, but the exact nature of the relationship is still being debated. It may also cause migraine headaches with aura, though evidence of the latter is scarce. As a result, there is no consensus on whether or not to close the flap.

“Some people feel very strongly that if you see a PFO, even if there are no associated problems with it, it should be closed at the time of surgery,” said study author Richard Krasuski, a cardiologist at the Cleveland Clinic. “I’ve heard surgeons say it’s just a simple stitch or two and the defect will be fixed. [These results make] you wonder, if you put a stitch or two in, what exactly are you doing physiologically?”

The researchers studied the medical records of 13,092 patients who had not been diagnosed with patent foramen ovale prior to surgery, from 1995 through 2006. The defect was discovered during surgery in 2,277 of them, or 17%.

Although that rate of diagnosis stayed steady during the 11-year study period, the rate of repair increased to almost 40% of diagnosed cases in 2003 despite a lack of evidence suggesting repair is beneficial, the study found.

The study showed that 1.2% of patients who didn’t have the defect repaired experienced a stroke, compared with 2.8% of patients who did. There was no difference in mortality.

Tim Gardner, medical director of the Center for Heart and Vascular Health at Christiana Care Health System in Wilmington, Del., said he generally repairs the defect when he finds it and was surprised by the study results. “The assumption has been that the procedure was safe and effective,” he said. “This report has certainly raised some concern.”

The increased rate of stroke could be a result of the closure procedure itself, in which stagnant blood flow in the atrium combined with surgical material and inflammation at the site of the operationcould make a blood clot more likely. Because of the open wound, the patient cannot take anticoagulant medication immediately.

Upcoming Military Robot Could Feed on Dead Bodies

It could be a combination of 19th-century mechanics, 21st-century technology — and a 20th-century horror movie.

A Maryland company under contract to the Pentagon is working on a steam-powered robot that would fuel itself by gobbling up whatever organic material it can find — grass, wood, old furniture, even dead bodies.

Robotic Technology Inc.’s Energetically Autonomous Tactical Robot — that’s right, “EATR” — “can find, ingest, and extract energy from biomass in the environment (and other organically-based energy sources), as well as use conventional and alternative fuels (such as gasoline, heavy fuel, kerosene, diesel, propane, coal, cooking oil, and solar) when suitable,” reads the company’s Web site.

That “biomass” and “other organically-based energy sources” wouldn’t necessarily be limited to plant material — animal and human corpses contain plenty of energy, and they’d be plentiful in a war zone.

EATR will be powered by the Waste Heat Engine developed by Cyclone Power Technology of Pompano Beach, Fla., which uses an “external combustion chamber” burning up fuel to heat up water in a closed loop, generating electricity.

The advantages to the military are that the robot would be extremely flexible in fuel sources and could roam on its own for months, even years, without having to be refueled or serviced.

Upon the EATR platform, the Pentagon could build all sorts of things — a transport, an ambulance, a communications center, even a mobile gunship.

In press materials, Robotic Technology presents EATR as an essentially benign artificial creature that fills its belly through “foraging,” despite the obvious military purpose.

H1N1 Swine Flu Appears Similar to 1918 Pandemic Virus; WHO Recommends Vaccines Use Live Influenza

Two shocking bits of news about the H1N1 swine flu virus emerged this week. The first is that the widely-circulating swine flu virus may be a lot more dangerous than people have so far been told: It appears to resemble the 1918 pandemic virus in the fact that it is capable of embedding itself deep in lung tissue and causing deadly infections. This is very different from the more common “seasonal flu” which does not replicate in the lungs.

Explaining this concept, lead researcher Yoshihiro Kawaoka of the University of Wisconsin wrote in the journal Nature: “When we conducted the experiments in ferrets and monkeys, the seasonal virus did not replicate in the lungs… The H1N1 virus replicates significantly better in the lungs.”

As Reuters reports, Kawaoka also added, “There is a misunderstanding about this virus… There is clear evidence the virus is different than seasonal influenza.”

The 1918 influenza pandemic killed at least 40 million people globally (and probably many more). Also remember: That was a time when obesity was extremely rare. Today, obesity is alarmingly common among first-world nations, and obesity is now suspected of making a person highly susceptible to swine flu fatalities

Meanwhile, the World Health Organization — always a trusty friend of Big Pharma — has announced that swine flu is “unstoppable” and is now directing all the nations of the world to vaccinate their citizens against the H1N1 influenza strain. This is essentially an order for global mandatory vaccinations.

And yet, at the same time the WHO is recommending mass global vaccinations against the swine flu, it also states on its own website that such vaccines have never been proven safe. In its own words:

“Since new technologies are involved in the production of some pandemic vaccines, which have not yet been extensively evaluated for their safety in certain population groups, it is very important to implement post-marketing surveillance of the highest possible quality. In addition, rapid sharing of the results of immunogenicity and post-marketing safety and effectiveness studies among the international community will be essential for allowing countries to make necessary adjustments to their vaccination policies.”

In other words: Use the people as guinea pigs and see who dies first!

It’s not difficult to decode the WHO’s language in this case. The vaccines aren’t safe, so they’re recommending that people given the vaccines be watched closely to see what happens (who dies).

And just to make sure more people die, the WHO is also recommending such vaccines use “live attenuated influenza.”

As stated on the WHO’s website:

“In view of the anticipated limited vaccine availability at global level and the potential need to protect against “drifted” strains of virus, SAGE recommended that promoting production and use of vaccines such as those that are formulated with oil-in-water adjuvants and live attenuated influenza vaccines was important.”

Bill Gates of Microsoft Envisions Fighting Hurricanes by Manipulating the Sea

If you thought domination of the world’s software market was cool, get a load of Bill Gates’ next technological vision: giant ocean-going tubs that fight hurricanes by draining warm water from the surface to the depths, through a long tube.

A second tube could simultaneously suck cool water from the depths to the surface.

Microsoft founder Gates and a dozen other scientists and engineers have a patent pending for deploying such vessels, which they say would collect water through waves breaking over the walls of the tub. Some variations have the water moving through turbines on their way down, which would in turn generate electricity to suck up the cooler water.

As many as 200 vessels could be placed strategically in the predicted path of a hurricane, and they could be designed to be reused or to sink in place and decompose underwater. The vessels could be moved into place by towing or by dropping from airplanes.

A second patent application describes how part or all of the cost of building and maintaining the hurricane-killer ships could be raised by selling insurance to coastal residents whose risk would be reduced by using the new system.

The hurricane-killing ideas, contained in a half-dozen related patent applications, were made public by the U.S. Patent and Trademark Office on Friday, with Gates listed as one of the inventors on each. The applications were submitted by Searete LLC, a subsidiary of Intellectual Ventures of Bellevue, Wash., and created by former Microsoft executives to both buy up existing patents and develop patent applications for new ideas.

The hurricane-killer system isn’t expected to be rolled out any time soon, however, according to a posting on the Intellectual Ventures Lab Web site.

Paul “Pablos” Holman, whose job title is listed as “hacker, ” said the system would be feasible only if other responses to more active hurricane seasons or more intense hurricanes caused by global warming do not work.

“This type of technology is not something humankind would try as a ‘Plan A’ or ‘Plan B, ‘ ” he wrote. “These inventions are a ‘Plan C’ where humans decide that we have exhausted all of our behavior changing or alternative energy options and need to rely on mitigation technologies.

“If our planet is in this severe situation, then our belief is that we should not be starting from scratch at investigating mitigation options, ” he wrote.

The water-moving vessels would not be limited to killing hurricanes, however. The applications also suggest the “wave induced downwelling” could stir up nutrient-rich sediment on ocean floors to promote plant and animal growth in environmentally-degraded areas.

“This may be used for developing wildlife preservation areas, wildlife recreation areas, restoring wildlife destroyed by natural or man-made causes, etc., ” according to the patent application.

Another proposal calls for moving nutrients and other material from the ocean floor to the surface to promote growth of algae to trap carbon as a tool in fighting global warming.

Intellectual Ventures was created in 2000 by Nathan Myhrvold, who was Gates’ chief technology officer at Microsoft, and Edward Jung, who was Microsoft’s chief software architect. In a May article on the unveiling of Intellectual Venture’s own patent laboratory, the Seattle Times reported that the firm has earned $1 billion in licensing revenue from patents it has acquired and about $80 million from patents for ideas it has created since its founding in 2000.

“We consider ourselves basically an invention business, ” said Marelaine Dykes, a spokeswoman for the company. “We’re a non-practicing entity, a non-manufacturing entity because we don’t produce products, per se.”

But the new laboratory does produce prototypes of some of its new inventions, she said, and subsidiaries like Searete are being formed to handle a variety of categories of the patents held or developed by the company.

The hurricane killing plan was the product of a gathering of scientists and invention developers more than a year ago.

“These are brainstorming sessions where we come up with and develop ideas around particular topics, ” she said. “We bring in smart people from all over, depending on the topic.”

Gates, an investor in Intellectual Ventures, has attended several of the invention salons, resulting in his name being added to the ensuing patent applications.

Others listed on those patents include a nuclear reactor designer, an aerospace engineer who has designed reusable launch vehicles, and a climatologist researching ways of increasing water droplets in upper-level clouds to reflect sunlight into space to fight global warming.

New Orleans residents might be keen on at least two other ideas for which Gates and his allies have sought patents: an insulated container that can be used as a beer keg and a fence using photons — particles of light — to shoo mosquitoes away from homes.

New Flu “Unstoppable”, WHO says, Calls for Vaccine

WASHINGTON (Reuters) – Saying the new H1N1 virus is “unstoppable”, the World Health Organization gave drug makers a full go-ahead to manufacture vaccines against the pandemic influenza strain on Monday and said healthcare workers should be the first to get one.

Every country will need to vaccinate citizens against the swine flu virus and must choose who else would get priority after nurses, doctors and technicians, said Dr. Marie-Paule Kieny, WHO director of the Initiative for Vaccine Research.

Several reports showed the new virus attacks people differently than seasonal flu — affecting younger people, the severely obese and seemingly healthy adults, and causing disease deep in the lungs.

Kieny briefed reporters on the findings of the WHO’s Strategic Advisory Group of Experts on Immunization, or SAGE. “The committee recognized that the H1N1 pandemic … is unstoppable and therefore that all countries need access to vaccine,” Kieney said.

“The SAGE recognized first that healthcare workers should be immunized in all countries in order to retain a functional health system as the virus evolves,” she added.

After that, each country should decide who is next in line, based on the virus’s unusual behavior.

Seasonal influenza is deadly enough — each year it is involved in 250,000 to 500,000 deaths globally. But most are the elderly or those with some kind of chronic disease that makes them more vulnerable to flu, such as asthma.

ELDERLY ADVANTAGE

The elderly seem to have some extra immunity to this new H1N1, which is a mixture of two swine viruses, one of which also contains genetic material from birds and humans. It is a very distant cousin of the H1N1 virus that caused the 1918 pandemic that killed 50 million to 100 million people.

A study published in the journal Nature on Monday confirmed that the blood of people born before 1920 carries antibodies to the 1918 strain, suggesting their immune systems remember a childhood infection.

The work by Dr. Yoshihiro Kawaoka also supports other studies that this new H1N1 strain does not stay in the nose and throat, as do most seasonal viruses.

“The H1N1 virus replicates significantly better in the lungs,” Kawaoka said. Other studies have also shown it can cause gastrointestinal effects, and that it targets people not usually thought of as being at high risk.

“Obesity has been observed to be one of the risk factors for more severe reaction to H1N1″ — something never before seen, Kieny added. It is not clear if obese people may have undiagnosed health problems that make them susceptible, or if obesity in and of itself is a risk.

On Friday, a team at the U.S. Centers for Disease Control and Prevention and the University of Michigan reported that nine out of 10 patients treated in an intensive care unit there were obese. They also had unusual symptoms such as blood clots in the lungs and multiple organ failure.

None have recovered and three died.

The CDC estimates at least a million people are infected in the United States alone and clinics everywhere are advised not to test each and every patient, so keeping an accurate count of cases will be impossible. The United States has documented 211 deaths and WHO counted 429 early last week.

Kieny said WHO would also work to get better viruses for companies from which to make vaccines. She said the strains that had been distributed did not grow very well in chicken eggs — used to make all flu vaccines.

One exception — AstraZeneca’s MedImmune unit makes a live virus vaccine that is squirted up the nose and it is easier to produce, Kieny said.

WHO said countries should continue with their normal vaccination programs against seasonal flu. Kieny said the seasonal H3N2 strain was also very active now in the southern hemisphere’s winter.

Sanofi-Aventis, Novartis, Baxter, Schering-Plough’s Nobilon, GlaxoSmithKline, Solvay, CSL and AstraZeneca’s MedImmune are among those working on flu vaccines.

U.S. Mulling Mortgage Aid for Unemployed

President Barack Obama is mulling new ways to delay foreclosure for jobless homeowners who are unable to keep up with monthly payments, an administration official said on Monday.

The official told Reuters it was reasonable for policymakers to consider options for loan forbearance — allowing borrowers to delay, defer or skip payments — that are more effective than those currently available in the private sector.

The number of failing home loans has been climbing for three years as risky borrowers have defaulted on their easy-to-get loans, property values have sunk and the unemployment rate has climbed.

But the official said the idea, which is still evolving, was difficult from a policy perspective and carries potential hazards. It could help more people struggling with economic difficulty, but it also could create perverse incentives that distort the housing market, said the official, who did not want to speak on the record about internal administration debates.

The official said such a program would be in keeping with other measures to help workers who have lost jobs in the current recession.

Officials have been frustrated as red-tape and rising mortgage rates have slowed a housing rescue plan announced in February that was meant to refinance 5 million borrowers and lower monthly payments for 4 million more.

A housing crisis of record defaults began at the end of a five-year housing boom of easy lending but the current crisis is being driven by climbing unemployment, say many analysts.

“All these numbers keep going up. We are not anywhere near the bottom,” said Jay Brinkmann, chief economist for the Mortgage Bankers Association.

Traditionally, homeowners have been tipped into default after a personal crisis, but the current downturn is worse as many borrowers have no home equity to soften the blow.

“What I don’t know is will every job loss, every divorce, every death going to lead to a foreclosure because there is just not enough equity left in the home to avoid foreclosure?” Brinkmann said.

Recent data from bank regulators present a mixed picture for the industry in responding to the foreclosure crisis as more modifications are being offered while the number of tardy loans continues to grow.

Servicers implemented 185,156 loan modifications during the first quarter of the year, up 55 percent from the prior quarter, according to data from the Office of the Comptroller of the Currency and Office of Thrift Supervision.

The report also showed that seriously delinquent mortgages, defined as loans that are 60 or more days past due, increased by nearly 9 percent from the prior quarter to 5 percent of all mortgages in the portfolio.

The Treasury Department asked the largest 25 mortgage service companies last week to appoint a special liaison officer to work directly with government officials trying to stem defaults.

Treasury will host a meeting with leading servicers on July 28 to hear how the companies are expanding their aid programs and making sure that those seeking help are not improperly disqualified.
