Antics of Drug Companies Disenchanting Population

I want to believe in America’s pharmaceutical companies. I want to believe that people in these companies believe that the best strategy for success is to do what is best for patients. I want to believe that they are interested in scientific truth and eager to know of any safety issues and ready to share that information with the public.

This week I was disappointed again.
Over the years GlaxoSmithKline ( GSK – news – people ) has repeatedly reassured the public about the safety of its blockbuster diabetes drug Avandia. But this weekend the Senate Finance Committee released a report revealing that inside the company Glaxo’s own experts and advisors were raising concerns about whether the drug could cause heart problems all along.

The report, based on more than 250,000 internal documents, provides a rare and unsettling glimpse into the decision by company executives to deflect safety issues–even as their own experts agreed with conclusions of outside researchers who were warning the public about possible harms.

The documents reveal that company researchers were deeply concerned about the cardiovascular safety of the drug as far back as 2003. The pages of the Senate report read like a spy novel: Glaxo receiving confidential documents leaked by a sympathetic academic who consulted for the company; the company embarking on a campaign to intimidate critics who warned about potential safety issues with the drug; and executives pulling strings to release data early from a scientific study that was supposedly controlled by an “independent” committee of researchers.

While Glaxo was publicly downplaying safety worries, a company statistician indicated that concerns raised by critics, including Cleveland Clinic cardiologist Steven Nissen, were legitimate. In one internal document the head of research states that analyses from the FDA, Nissen and GSK all suggested that Avandia could be causing heart attacks. Meanwhile, Glaxo’s media relations department was telling the public that there was no link between the drug and heart disease.

Scientists Warn of Fraud of Stem Cell ‘Banks’

Clinics that offer to “bank” stem cells from the umbilical cords of newborns for use later in life when illness strikes are fraudsters, a top US scientist said.
Clinics in many countries allow parents to deposit stem cells from their neonate’s umbilical cord with a view to using the cells to cure major illnesses that could occur later in life.

In Thailand, for example, parents pay in the region of 3,600 dollars to make a deposit in a stem cell bank, thinking they are taking out a sort of health insurance for their child.

But Irving Weissman, director of the Institute of Stem Cell Biology and Regenerative Medicine at Stanford University in California, said the well-meaning parents were being fleeced by the stem cell bankers.

“Umbilical cords contain blood-forming stem cells at a level that would maintain the blood-forming capacity of a very young child,” Weissman told reporters at the annual meeting of the American Association for the Advancement of Science (AAAS).

“They could also have derived mesenchymal cells — fiberglass-like cells that have a very limited capacity to make scar, bone, fat — but they don’t make brain, they don’t make blood, they don’t make heart, they don’t make skeletal muscle, despite what various people claim,” he said.

Weissman said these “unproven stem cell therapeutic clinicians” tend to set up shop in countries with poor medical regulations, but AFP found websites for umbilical cord stem cell banks in European Union member states and in the United States.

“They do the therapies, then they let the patients go on their own, short of maybe 50-150,000 dollars for a therapy that has no chance — taken away from a family that needs them when they have an incurable disease,” Weissman said.

“It is wrong.”

The International Stem Cell Society is due to issue a report in April about unproven stem cell therapies such as banking a baby’s umbilical cord blood for future use.

Heart Transplants Linked to Skin Cancer

A new study published by the Mayo Clinic in the journal Archives of Dermatology has revealed that roughly half of all patients who undergo a heart transplant end up getting skin cancer within 15 years of the surgery. Among the 312 people evaluated in the study, 1,395 different skin cancers emerged following surgery.

Whenever a body part is surgically implanted, the body recognizes it as a foreign object and proceeds to attack it. Patients who undergo organ transplants usually have to take immune suppressant drugs for the rest of their lives in order to prevent the body from rejecting the organ. These drugs increase patients’ risk of developing cancer and other serious diseases.

The Mayo Clinic study evaluated what types of skin cancers emerged following a heart transplant and found that all sorts of skin cancers, from minor to fatal, afflicted patients. While relatively rare, researchers discovered that some patients developed malignant melanomas, the most serious type of skin cancer. One study participant died from malignant melanoma.

A 2005 study published in the journal CANCER revealed that patients who undergo kidney transplants are four times more prone to getting melanoma. As patients age, that risk continues to increase due to the long term immuno-suppressive action of the drugs. Other studies have found that organ transplant patients are at a higher of developing all cancers, not just skin cancer.

Many doctors are encouraging patients who have undergone transplant surgery to perform self evaluations on a regular basis to identify the presence of skin cancer. By finding it early, the majority of skin cancers can be eradicated. Some also recommended that post surgery patients be screened regularly to help catch a developing cancer as early as possible.

Amazingly, some experts believe that the increased cancer risk following surgery highlights the amazing success of contemporary transplant surgery techniques. Their logic concludes that since surgery prolongs a person’s life, those extra years allow additional time for future problems to materialize that would have otherwise been unable to.

While it may seem like a great success to have developed advanced ways of transplanting organs, many may argue that having to take potentially life-threatening drugs in order to keep the organs functioning is actually an ironic failure. Not every transplant patient will end up getting cancer but a great many will live out the rest of their days in poor health due to severe drug side effects.

Chemicals Pass Through Breast Milk to Cause Cancer

Higher exposure to toxic chemicals may explain the difference in testicular cancer rates between Denmark and Finland, researchers from the University Department of Growth and Reproduction have found in a study on breast milk.

“Our findings reinforce the view that environmental exposure to [endocrine-disrupting chemicals] may explain some of the temporal and between-country differences in incidence of male reproductive disorders,” said lead researcher Niels Skakkebaek.

Rates of testicular cancer, genital abnormalities, low semen quality, and other male reproductive disorders are four times higher in Denmark than in nearby Finland. These conditions have previously been linked to exposure to industrial chemicals that disrupt the hormonal (endocrine) system.

Endocrine disruptors have also been linked to birth defects, neurological problems, and increased rates of cancer and heart disease. The most dangerous chemicals are known as persistent organic pollutants, because they resist environmental degradation and accumulate in the environment.

Most of these chemicals bind to animal fat. As a consequence, animal-based foods tend to contain higher concentrations. So does human breast milk.

In the current study, researchers tested the breast milk of 68 women in Denmark and Finland for 121 different chemicals. They found significantly higher levels of pesticides, dioxins and polychlorinated biphenyls (PCBs) in the Danish breast milk.

The higher rates of testicular cancer and other reproductive disorders in Denmark may not be explained directly by contamination via breast milk. Breast milk contamination is thought to be a reliable marker of prenatal chemical exposure, which is likely to pose an even greater risk.

Canadians Having Surgery in U.S. – Raising Questions on Health Care

At the center of controversy is Danny Williams, premier of Newfoundland and Labrador. Williams’ decision to head south across the border for his surgery is drawing fire from defenders of the Canadian health-care system – a favorite example for proponents of a government-run health care in the U.S.

Williams, a millionaire and former lawyer, left Canada on Monday to seek treatment at an unspecified hospital in the U.S. It is not clear what kind of surgery he’ll undergo, though Newfoundland Deputy Premier Kathy Dunderdale said that having the surgery in the province was not an option.

So what about a hospital in Toronto, Montreal or Vancouver?

“Virtually all forms of cardiac surgery are looked after in Canada, and I would say extremely well,” Dr. Chris Feindel, a cardiac surgeon at Toronto’s University Health Network told the National Post. “Personally … I would have my cardiac surgery done in Canada, no matter what resources I had at my disposal.”

Feindel was quick to point out that U.S. patients have come to UHN’s Peter Munk Cardiac Centre for valve repairs.

Canadian Sen. Wilbert Keon, a retired heart surgeon and professor emeritus at the University of Ottawa, told the Toronto Sun that Newfoundland does not have the kind of “post-surgery technical support to allow all advanced complicated procedures to be performed there.”

But, “I can’t imagine anything that couldn’t be done in Canada that is done in America,” he told the newspaper.

“Virtually all” complex heart surgeries could be performed Ottawa’s Heart Institute, as well as medical centers in Toronto, Montreal and Edmonton, Keon told the Sun.

Dr. Marc Siegel, an internist and Fox News contributor disagreed with Feindel and Keon.

“You would not find a U.S. governor going to Canada for surgery,” Siegel said. “We’d be putting our quality of care at risk if we went to a single-payer system like Canada.”

Siegel said if the U.S. does move to a universal health care system, people wanting the “crème de la crème” in medicine would probably have to go elsewhere.

“It is quite possible that the procedure he is getting is so specialized that the top doctors doing it are to be found only in the U.S.,” Siegel said.

There also is the possibility that Williams has private health insurance, which may or may not have been accepted at Canadian hospitals.

“I would expect that he is eligible for all the rest of us would be in terms of our own private insurance or government insurance, and I’m sure there’s anything over and above that, the premier would certainly take care of it himself,” Dunderdale told the Press.

Williams’ recovery is expected to take three to 12 weeks, Dunderdale said.

20% of American Teens Have Unhealthy Cholesterol

One in five American teens has unhealthy cholesterol levels, a major risk factor for heart disease in adults, the U.S. Centers for Disease Control and Prevention said on Thursday.

The heavier teens were, the more likely they were to have high cholesterol but even 14 percent of teens with normal body weight were found to have unhealthy cholesterol levels, the CDC said.

CDC researchers studied data on 3,125 teens collected from the National Health and Nutrition Examination Survey for 1999 through 2006.

They found that 20.3 percent of young people aged 12 to 19 and more boys than girls had unhealthy cholesterol levels.

The study found that, based on American Academy of Pediatrics guidelines, a third of teens would be eligible for cholesterol screening based solely on being overweight or obese.

The AAP also recommends screening for young people who have a family history of high cholesterol or premature heart disease.

The researchers analyzed measurements of low-density lipoprotein — LDL or so-called bad cholesterol; high-density lipoprotein, the HDL or “good,” cholesterol; and triglycerides.

Bad cholesterol can help clog arteries while good cholesterol carries away the bad stuff. People should aim for low LDL and triglycerides and high HDL.

Ashleigh May of the CDC, who led the study, said the results were “very concerning.”

“It’s a large proportion of the youth that have at least one abnormal lipid level. That is concerning given the long term implications for heart disease,” May said in a telephone interview.

Unhealthy cholesterol levels, which often begin during childhood and adolescence, are a big risk factor in heart disease, the No. 1 cause of death among adults in the United States.

“We really want to make sure that clinicians are aware of lipid screening guidelines and lifestyle interventions that are recommended, for youth, especially overweight and obese youth,” May said.

“For all youth, healthy eating habits and physical activity are good ways to reduce their risk for abnormal lipids and heart disease in the long term.”

Merck Withheld Key Vioxx Data

A recent study published in the Archives of Internal Medicine has revealed that information about heart risks from pharmaceutical giant Merck’s Vioxx drug was available in 2000, four years before the Merck pulled the drug from the market. Because the information was not published and made public, Merck sat on it until a later clinical trial openly revealed that the drug was causing strokes and heart attacks.

Dr. Harlan Krumholz, study author from the Yale University School of Medicine, noted that he obtained pertinent safety data about Vioxx only after a lawsuit was filed against Merck by those who had been injured by the drug. It was discovered that out of the 30 studies conducted by Merck prior to when Vioxx was withdrawn, only 18 of them had been published. Six were published after the drug was withdrawn and six were never published at all.

After mulling through the study data, one trial at a time, Krumholz and his team clearly identified a link between Vioxx usage and increased heart attacks and strokes in patients. Based on when the studies were conducted, the connection was visible as early as December of 2000.

Ron Rogers, a Merck spokesman, denied the claims that any link could be observed and decried the methods used by researchers to come to this conclusion, despite acceptance of the findings following a rigorous peer review process. Rogers stated that the company’s own extensive analysis showed no connection between Vioxx usage and increased cases of heart attack and stroke prior to the time when it was removed from the market, emphasized that the company had no prior knowledge of Vioxx’s dangers.

However in 2004, the Wall Street Journal (WSJ) reported that it had seen internal Merck emails exchanged between company executives that expressed concern over Vioxx’s tendency to increase the risk of heart attack. The entire series of emails clearly indicated that Merck knew about the dangers of Vioxx and was doing its best to conceal the information.

Dating back to the late 1990s, early emails contained dialogues about how to craft a study that would minimize the truth about Vioxx. An email from March of 2000 sent by Merck’s research chief, Edward Scolnick, expressed clear affirmation that heart problems associated with Vioxx were “clearly there” and that it was a “shame.”

When questioned about the emails, Merck once again denied the allegations, claiming that the emails were taken out of context. Merck never provided an explanation as to what the emails were referring to in their supposed proper context.

Thousands of injured patients and company shareholders filed a class action lawsuit against Merck following its removal of Vioxx from the market. Merck appealed the lawsuit on the grounds that “sufficient” information about the drug’s risks were available when the drug hit the market. Merck succeeded in convincing a U.S. district judge to dismiss the lawsuit because it was filed after the two-year statute of limitations ended.

However an appeals court in Philadelphia reversed the decision on behalf of the many shareholders who lost a great deal when Vioxx was suddenly removed from the market, which caused Merck’s stock values to plummet. Since it was determined that shareholders could not have known what was coming based on the information that was made publicly available, the Supreme Court is going to evaluate the case and make a decision on it next year.

Merck also agreed to a $4.85 billion settlement, one of the largest in history, on behalf of the thousands who filed personal injury lawsuits against the company due to serious injuries caused by Vioxx. (These cases were different from the ones included in the initial class action suit). The drug giant said it agreed to the settlement because the litigation process would have taken countless years to resolve, most likely hurting the company’s reputation even further.

At the very least, drug safety tracking once receiving approval from the FDA to go to market needs a major overhaul. Dr. Krumholz and his colleagues stressed this point following their Vioxx discoveries. When fraud and criminal behavior are involved, as has shown to be the case with Merck, justice must be served.

The Vioxx scandal illustrates an important fact about the drug industry in general. Big Pharma continually gets away with massive impropriety. Its business practices, from research and development to marketing, are wrought with dishonesty, manipulation and downright fraud. There is arguably no other industry that gets away with its crimes as much as the pharmaceutical industry does. The consequences are also the most severe, costing millions of people their health and oftentimes their lives.

Bringing the issue to light as often and loudly as possible will only go so far. Massive reform, in some way, shape, or form, must be implemented if there is ever going to be an end to the madness.

Monsanto Corn Linked to Organ Failure

In a study released by the International Journal of Biological Sciences, analyzing the effects of genetically modified foods on mammalian health, researchers found that agricultural giant Monsanto’s GM corn is linked to organ damage in rats.

According to the study, which was summarized by Adam Shake at Twilight Earth, “Three varieties of Monsanto’s GM corn – Mon 863, insecticide-producing Mon 810, and Roundup® herbicide-absorbing NK 603 – were approved for consumption by US, European and several other national food safety authorities.”

Monsanto gathered its own crude statistical data after conducting a 90-day study, even though chronic problems can rarely be found after 90 days, and concluded that the corn was safe for consumption. The stamp of approval may have been premature, however.

In the conclusion of the IJBS study, researchers wrote:

“Effects were mostly concentrated in kidney and liver function, the two major diet detoxification organs, but in detail differed with each GM type. In addition, some effects on heart, adrenal, spleen and blood cells were also frequently noted. As there normally exists sex differences in liver and kidney metabolism, the highly statistically significant disturbances in the function of these organs, seen between male and female rats, cannot be dismissed as biologically insignificant as has been proposed by others. We therefore conclude that our data strongly suggests that these GM maize varieties induce a state of hepatorenal toxicity….These substances have never before been an integral part of the human or animal diet and therefore their health consequences for those who consume them, especially over long time periods are currently unknown.”

Monsanto has immediately responded to the study, stating that the research is “based on faulty analytical methods and reasoning and do not call into question the safety findings for these products.”

The IJBS study’s author Gilles-Eric Séralini responded to the Monsanto statement on the blog, Food Freedom, “Our study contradicts Monsanto conclusions because Monsanto systematically neglects significant health effects in mammals that are different in males and females eating GMOs, or not proportional to the dose. This is a very serious mistake, dramatic for public health. This is the major conclusion revealed by our work, the only careful reanalysis of Monsanto crude statistical data.”
